What factors contribute to MISS rate for spells and melee?

I see the means for passive defenses (shield, parry, evade), but I don't know the mechanics to affect a MISS, whether standard DAoC / 1.65 or anything custom to Phoenix. I am interested in both causing my opponent to miss and reducing my own chance to miss.

Armor Factor by armor, absorption, or spells seems immediately likely. How about stats (dex? quick?), weapon skill, or passives? I see some remarks about Defense Penetration on multiple freeshards, but not a lot of detail about how it works or what it does (such as, is this mechanic even related to MISS rates). I'm sure level is a factor as level is deeply part of most DAoC mechanics, and if formulae exist I'd like to know them, but I'm mostly concerned about 50v50.

18% base miss
- style to hit bonus
- weapon bonus (enchanter)
+ enemy armor bonus (enchanter)
+ enemy style defensive bonus

Here:
18% base miss
- style to hit bonus
- weapon bonus (enchanter)
+ enemy armor bonus (enchanter)
+ enemy style defensive bonus
- 0.05% per weapon spec

Weapon / armor bonus should cancel each other out unless someone forgot to go to the enchanter.

Just to confirm, "- 0.5% per weapon spec", that's 0.5% per point of weapon spec of the style used to attack, or is it per point of base weapon spec only, regardless of style line used to attack?

0.05%, and it's the higher of base weapon spec or style spec (if a style is used), including +from items and rr

Is this information accurate for Phoenix regarding style to-hit bonus?

"Style bonus is added to your to-hit chance according to the following table (defensive penalties are added to your opponents to-hit chance) :
very high bonus: 20%
high bonus : 15%
medium bonus : 10%
low bonus : 5%"

How about from spells? I would technically "resist" a spell effect rather than it missing, but the word "Resist" is a bit overloaded for also reducing the damage / effect / duration of the spell, so I've used the word MISS instead. Whatever term you prefer for causing a spell to not affect the target at all.

Scrolling through combat logs, the listed % chance for my opponent to miss a melee attack is about twice as high as the % for "you resisted the effect."

Also, are there any offensive spells that would always connect, similar to stealth opener styles from Critical Strike?
