I have a friend trying to play the game (first timer) he doesn't like how small the UI is in 1440p. Is there a custom UI that will increase the size of all the hotkeys, etc?

No, there is not.
As far as I know scaling the buttons/quickbars would be a ton of work as the size is somewhere stored in the client, no one is going to do that.
Only possibility is to scale the screen size in windows down to i.e. 1920x1080 while playing. I use this resolution and the icons/UI are OK.

if the friend is on windows 10 he can change the scaling and daoc will scale with it.
bigger % means lower resolution in daoc so it will be bigger UI.

Only way I was able to make the UI bigger was drop resolution to 1440x900. It helped. Also increase font side in chat windows. All that without custom ui.
Also, adjust lighting if you use hdmi connection otherwise its too dark to see.

For people like me having a high resolution screen, I found a solution using two free software (perhaps there are better software and one software that can do it all) but anyway they are called "Sizer" and "ResizeEnable". For example in daoc I set my resolution as windowed 800*600 and in Sizer I set the double of this resolution (to keep aspect ratio) so 1600*1200 and then right click daoc windows corner select the custom resolution previously created and it zoom perfectly (you need ResizeEnable to be able to select window border).
